To assign your United Healthcare commissions to another agent, you will complete the UHC Assignment of Commission Form.

This form should be completed by the writing agent and submitted to sh_commissions_administration@uch.com and cc to medicare@lifeprofessionalsseniorsolutions.com for processing.
The "FROM" section is where you will place the information of the agent who is writing the business.
The "TO" section is where you will place the information of the agent who the commissions will be paid to.
