Humana Agent Business Transferal Form (ABTF)
To assign your Humana commissions to another agent, you will complete the Humana Agent Business Transferal Form.
Section 1: Agent Information
This section will be the information for the agent who is writing the business. Keep in mind that the Humana Agent Number/SAN is the writing number that Humana provided. Complete this entire section with the information for the agent who is writing the business:
Section 2: Business Type for Assignment of Commission
This section will be where the writing agent selects which type of business will be assigned to the new Agent/Agency to be paid.
Please complete each type applicable and select whether or not it will be for Future Business Only or Existing & Future Business.
Section 3: Signature of Writing Agent
This section is where the writing Agent will sign and date for approval of the Assignment of Commissions.
Once the form has been completed, email to agencymgt@humana.com and cc to medicare@lifeprofessionalsseniorsolutions.com for processing.
